New Staff / Full-Time Faculty Set Up Request

In addition to completing this form the Office of Human Resources will request your new employee:

  1. Provide documents that will satisfy employment eligibility and identity as established on Form I-9.  This documentation must be provided within three (3) days of the employment date.

  2. Provide financial institution information for completion of Direct Deposit form for Payroll, including a voided check.

  3. Make sure an employment application is completed and signed.

  4. Provide a certified copy of his/her college transcript(s) for your highest degree if your new employee is a faculty member.  The official transcript(s) should be sent to the Office of Academic Affairs prior to the first day of employment.
